Every order includes two pad of Waste Witness Verification forms. Put one copy in the patient file and one copy with your controlled substance files. The requires records/tracking every mg/ml of a controlled substance from its creation to end-user (patient) or proper destruction. These convenient forms allow you to account for wasted amounts.
